Registration: By my acceptance of this Player Agreement as indicated below, the above-named player is hereby registering to try out for and/or play ice hockey in a program offered by Greater Boston Hockey, LLC (the “League”). I understand that my registration is not effective until accepted by the League, and that the League may decline to accept my registration for any reason not prohibited by applicable law. Upon acceptance of my registration by the League, this Player Agreement becomes a contract under which the League agrees to permit me to participate in the League’s programs subject to and in accordance with the League’s rules and this Player Agreement. I agree that this Player Agreement is binding upon me throughout the duration of my membership in the League and applies to any evaluation skate, pick-up skate, ice hockey game, or any other League sponsored event. Unless the League notifies me to the contrary, I understand that my registration is deemed accepted by the League upon the League’s receipt of this Player Agreement together with the required payment in full or participation in the League’s installment payment plan. The League reserves the right to terminate any player’s participation in League activities at any time during the course of that activity, session or season, for any reason not prohibited by applicable law.

Assumption of Risk and Release of Liability: I understand and accept the fact that ice hockey is a hazardous sport, and I realize that injuries are a common occurrence. I agree, as a condition of being allowed to participate in League activities, that I freely accept and voluntarily assume all risks of personal injury or death or property damage, and release Greater Boston Hockey, LLC, its owners, agents, employees, directors, officers and members and their successors in interest, from any and all liability for personal injury or property damage which results in any way from negligence, conditions on or about the ice and/or rink, the operation of the League including, but not limited to, practice and game play, ice maintenance, actions or omissions of employees or agents of the League, or my participation in ice hockey, accepting myself the full responsibility.

Refunds: I understand and agree that once I am selected in the League’s draft or two business days prior to the League’s first game, whichever comes first, the League will not refund any fees paid for any reason.

Fighting: I acknowledge that fighting or any attempt to injure another player has no place in the League. I understand that fighting will result in my expulsion from the League.

League Rules: As a condition of my participation in League activities, I agree to abide by all rules, policies, directives, or decisions of the League, whether presently in force or adopted by the League in the future, including the League’s rules that all players:

1. must wear HECC approved protective helmets, and other protective equipment at all times while on the ice surface, player’s bench or penalty box, whether or not play is under way. I am aware that for safety reasons, the League strongly recommends that all players wear an approved full facial mask;
2. shall conduct themselves in a mature and responsible manner which will reflect favorably upon the League;
3. shall never participate in any game or League activity while under the influence of alcohol or any drug or intoxicating substance, and will abide by ice facility alcohol policies in effect.
4. may be expelled without refund from the League for failure to comply with this Player Agreement, any League rule, directive, or decision, or for any of the following reasons:
   1. failure to make full payment when due;
   2. falsification of registration information;
5. The rules and regulations of the League are subject to change, without notice, as published from time to time in any subsequent version(s) of the League’s Player Agreement, Terms of Registration and/or the Official League Rules.

Additional Terms and Conditions: I further understand and agree to the following:

1. For and in consideration of my registration and participation in League programs and/or activities, I hereby agree to hold harmless the League, member teams, event hosts and each of them, their directors, officers, employees, operators, trustees, members, and agents for any and all accidents or loss, however caused.
2. The League’s decision shall be final on the adoption, enforcement, and interpretation of all League rules and policies, and all other matters relating to the League’s activities. I agree to abide by all such decisions.
3. The League will be the sole authority as to what team I will be permitted to participate.
4. Each team will have a team captain. The team captain will be authorized to act as the sole agent of the team for all purposes relating to League activities and events. If I become a team captain, I will act to ensure that the team strictly complies with all of the League’s rules and policies.
5. Jerseys are the property of the League. I will return my jersey within two business days of my final game or I agree to pay a $50 replacement fee. Any jersey that is lost, damaged in excess of normal wear and tear, requires special cleaning, or is contaminated with blood will be considered as not returned, and I agree to pay the $50 replacement fee.
6. The League may use for promotional, advertising, or educational purposes, without notice or compensation to me, any audio and/or visual recordings of any League activity in which I may appear.
7. “Draft Hockey League”, “2009 Draft Hockey League”, and the names of and rights to all the League’s teams, are the exclusive property of Greater Boston Hockey, LLC and shall not be used in any manner in connection with any non-League play that has not been expressly approved or endorsed by the League.
8. This Player Agreement, in conjunction with the League Rules and any written notification of playing rules, policies, or conditions, represents the entire agreement between the League and me, and there are no other representations, inducements, warranties, promises, or agreements, oral or otherwise, between the League and me that are not set forth above. This agreement may be amended or modified only by writing signed by me and an authorized agent of the League. To the extent there exists any inconsistency or conflict between this Player Agreement and any other statement of League rules, this Player Agreement shall prevail and shall remain fully binding upon me.
9. This Player Agreement shall be deemed executed in the Commonwealth of Massachusetts and shall be governed and interpreted by the laws of the Commonwealth of Massachusetts.
10. This Player Agreement may be executed by facsimile transmitted signature or online acceptance and all parties agree that the reproduction of a signature by way of a facsimile transmitted device or electronic record of acceptance produced by the League will be treated as though such reproduction was an executed original.

I have read, understand and agree to be bound by this Player Agreement.
